Residential Care
The Department of Health and Social Services (HSS) offers residential care services for people who need daily assistance due to physical or mental illness, or old age. People who are not able to take care of themselves or cannot be cared for in their own home are eligible for residential care services.
Residential care facilities offer safe and supervised living arrangements that include housing, meals, medical care, and help with daily activities.
Residential care services may be temporary or permanent. Clients in residential care facilities can stay until their condition improves, or until they need to be moved to another facility that can offer more support.
Eligibility
To be eligible for residential care services, you must be enrolled in the Nunavut Health Care Plan and get a referral from your local social worker.
Your local social worker will complete a Social Services Care Assessment to find out what kind of support you require and what facility would be best for you.
The Social Services Care Assessment will fit you into one of the following five levels of care, which will show what kind of support you might need.

Long Term Care Facilities
HSS offers Continuing Care Centres in Gjoa Haven and Igloolik that provide long term to Nunavummiut.
These facilities provide nursing care for people that need on-going support for chronic conditions or end of life care.

Getting Ready for Residential Care
Once a client is referred to Residential Care and a facility has been chosen, the Department of Health and Social Services will make arrangements for medical travel to the facility for the client and an escort.
Clients should bring items that will make their living space at the facility feel like home. They can decorate their own living space.
Clients should also bring any clothes or personal items that they would like to keep with them.
At the facility, workers will make a personal care plan for the client stating the goals of the residential care placement. Workers will review this plan once a year to make sure that the plan meets the client’s needs.
When a client enters a residential care facility, the family is encouraged to keep in contact with the client.
Families of clients living in residential care facilities can speak to a local social worker for information on keeping in contact with the client. Social workers can also give the family regular progress reports about the client.
